Health Insurance Plan - Taking care of you and
your family At Spillman, health care and wellness are
priorities for our employees. Our plan offers many
programs to ensure wellness at NO cost to the employee.
Employees are eligible for health coverage the
first of the month following 30 days from the date
of hire. To help contain costs, Spillman's plan
allows employees to participate in health care
networks that provide high-quality care at a reasonable
cost. Use of the networks is voluntary and means
lower health costs for Spillman employees. Plus,
our plans have NO individual or family deductibles
when you stay within the network. Possible out-of-pocket
expenses include:

Dental Plan - Taking care
of you and your family Dental coverage won't ease the pain of a
root canal, but it will ease the financial burden
and help improve your family's dental health. Our
plan pays 100 percent of preventive care and shares expenses
of other qualifying dental treatment. It also includes
orthodontic coverage for dependents under age 19.

Vision Plan - Taking care
of you and your family Each person on the Vision Plan receives
coverage for a vision examination and either one
pair of eyeglasses (lenses and frames) or one pair
of contact lenses every two years.

Life Insurance Plan - Providing
more coverage AND more choices Spillman pays the premium for $50,000 in
life insurance for the full-time employee, $5,000
for spouse, and $2,000 for each dependant child.
Children are covered from live birth to age 26.
Additional coverage can be purchased for the employee
or other family members.

401(k) Retirement Plan
- Putting you in charge of your future Spillman's matching-funds 401(k) retirement
plan lets you control how much of your paycheck
you save, within government guidelines. Spillman
contributes up to 4 percent of your income when you save
at least 6 percent of your income. You are eligible for
this benefit upon completing six months of full-time
employment. Contributions and earnings are tax-deferred
(exempt from federal, state, and local income taxes)
until you take a distribution from the plan. Vesting
occurs at 20 percent per year. The plan gives you various
investment choices, which you can change to meet
your needs. Participating in Spillman's retirement
program is an easy, strategic, long-term method
of saving and investing for your future.

Long-term Disability Plan
- Providing financial protection when you can't
work Should a medical condition prevent you from
working longer than three months, you can continue
to receive a portion of your salary under Spillman's
long-term disability plan. This plan pays 60 percent of
the monthly earnings, to a maximum benefit of $10,000
per month. You are eligible to participate in the
long-term disability plan 91 days after beginning
full-time employment.

Short-term Disability Plan
- Providing financial protection when you can't
work Should a medical condition prevent you from
working, you can continue
to receive a portion of your salary under Spillman's
short-term disability plan. This plan pays 60 percent of
the monthly earnings. You are eligible to participate in the
short-term disability planon the first day of an accident or the eighth day of an illness or hospitalization.

Employee Assistance
- Taking care of you and your family The Employee Assistance Program provides
confidential, short-term counseling to help employees
and family members resolve personal problems. This
service is available at no cost to employees.
